This Steering Committee was initally formed to ensure marine operations participation in the Marine Step Change Initiative. This involvment is enacted through the Party Cheifs and Captains Network (PCCN). As participation and attendance at their bi-annual meetings has been strong, the Steering Committee itself has not been reconstituted. Should a time come when interest or involvment in the PCCN wains, the Committee will again be formed to enforce Marine Step Change action.

Link to a Step Change database called SADIE. SADIE - the Safety Alert Database and Information Exchange -is a free web-based service jointly sponsored by the Health and Safety Executive (HSE) andthe UK Offshore Operators Association (UKOOA).Although conceived primarily as a safety tool for the offshore industry, the database is freely availableto anyone who feels they might benefit from access to the information.

Aide Memoir for Geophysical Risk Assessors

What:   
A call for support and information: A simple database of fatalities that have occurred in world wide Geophysical operations.

Risk management has to be experience-based, and hence needs a “memory function.”  Risk assessments for low frequency, high potential incidents (fatalities) need to be done on the basis of collective industry experience.  With the great crew-change that is occurring today, to an extent this memory is being lost.

To help address this situation, IAGC and the OGP Geophysical HSE Subcommittee are assembling a simple database of fatalities from geophysical operations that have previously occurred.  We believe this important project will improve the industries’ HSE management of geophysical operations.  The database would serve this purpose and furthermore:
 
 
  • Provide essential training material for the new generation of professionals.
    • Knowledge of what has gone wrong in the past.
    • Avoid re-inventing of the wheel the hard way.
  • Help answer the question: “Has this happened before?”
  • Provide completeness checks on hazard registers and HSE guidelines and manuals.
  • Provide further statistical insight on the risk pattern and profile for Geophysical operations.
  • Enable the use of the full industry experience, rather than the limited experience of a single company or individual.
What has been done so far?
Sources so far used have been OGP annual reports, IAGC, CAGC and Step Change (SADIE) databases and reports from a number of companies and experienced individuals.  To date from these sources we have captured the following, all of which is available by downloading this spreadsheet.  Information on 550 fatalities, from 348 accidents in Geophysical operations.
  • In addition, we have included 324 fatalities from 221 accidents that happened in other E&P operations, but could have easily happened in Geophysics. 
While we believe this is far more than just a tip of the iceberg, we are convinced that a lot more fatal accidents have happened than what we have so far captured (and from which we all could learn a lot more).
 
How can you help?

We request that companies and/or individuals send us information on additional incidents not yet collected in our database. 
 
 
 
Please have a look at the database structure and content and send us any further information you may have related to fatal accidents on which you or your company have direct information. 

NEW - We have added a User Manual (first draft) to help one use the database and in providing additional items to the database.  [Thanks to Eelco for preparing this!] 

Details:
  • Information should be sent to IAGC's Tim Fransioli or to the OGP via Mr. Eelco Sixma.
  • The database is kept fully anonymous, with no country, company, contractor or victim information.  Only the year of the accident is recorded, to reduce the possibility of entering duplicate events.
  • We are not asking for formal accident reports (although those would be best).  In fact, we accept data from “memory,” provided the source has first hand knowledge of the event, i.e. we must avoid “hear say.”
  • We have now also included the vehicle incidents from the source data bases.
  • In due course, we plan to package this as a joint OGP / IAGC report.
  • The spreadsheet database will also be made available to the industry via the web (see links).
  • We intend to periodically update the database.

We all owe it to the victims of these tragic accidents that we do remember what happened and prevent this from happening again.  Please help your industry do this!

ASA Plans to Develop Towed PAM Standard

The Acoustical Society of America (ASA) has requested support to prepare an American National Standard Institute (ANSI) standard on towed cabled passive acoustic monitoring (PAM) systems for monitoring and mitigation purposes. If this moves forward, IAGC will have a role on the working group.

The ASA PAM standard would cover requirements and recommendations for initial planning (including guidelines for when PAM is not appropriate for a planned field operation), hardware, software, training, real-time mitigation procedures, and performance validation, but would not cover operational shutdown decision criteria, sound source verification, or how to establish the required detection range of the system.

ASA’s motivation for developing this standard is that several regulatory agencies recommend using towed PAM for real-time mitigation and monitoring during seismic surveys and in some instances, require its use. However, to date, no specific guidelines have been developed or implemented.

ASA proposes that the working group would include representatives of a major oil company, an acoustic consulting company, IAGC, and each US federal organization that has an interest in supporting the standard.

As part of its 2013 work, the IAGC Sound & Marine Life (SML) Workgroup will be developing a PAM guidance document for IAGC members. The SML Workgroup will ensure that this guidance document is in alignment with proposed ASA standard.

 

Land & Marine Safety Manuals Version-10 Printed

The latest versions of the land and marine safety manuals (English only) have been printed by the publisher. Orders should be placed now for earliest delivery.

As noted in a previous newsletter article, IAGC will have a limited number of both manuals (600 marine and 400 land) delivered via air freight, which we are expecting to receive in late January. The remainder of the manuals will be shipped via sea/land and will arrive in Houston late February or early March.

The cost of printing and shipping has increased since the last versions of the manuals were published, so the cost per manual for the Version-10 land and marine safety manuals will be more. Land and Marine safety manuals will cost USD 15 for members and USD 18 for non-members. If you would like to place an order for safety manuals, please contact Tim Fransioli (IAGC Website and Data Base Administrator), and when the manuals arrive in Houston, your order will be processed. 

 

Spring 2013 IAGC / OGP Joint HSE & Security Forum – Save the Date!

Mark your calendar and plan on attending the Spring 2013 IAGC/OGP Joint HSE & Security Forum, which will be held in Oklahoma City, OK, USA, on the 23rd& 24th of April.

Dawson Geophysical is hosting the Forum at the Cox Conference Center. Vicki Huebler – ION (HSE & Security Steering Committee Chair) will be meeting with Randy Ingram – ExxonMobil (OGP HSSE Geophysical Subcommittee Chair) and Mark Nelson – Dawson Geophysical later this month to finalize the agenda for the Forum, which will include company presentations of high potential incidences, case studies highlighting best practices, as well as updates from IAGC and OGP member company representatives on the various HSE & Security related activities occurring within the two Associations. Vicki, Randy and Mark will be identifying topics and speakers over the next few weeks to fill out the agenda. If you have any suggestions, you are encouraged to contact Vicki, Randy, Mark or Walt Rosenbusch (IAGC).  A draft agendawill be made available on the IAGC website in February.

Dawson has made arrangements with the Renaissance Hotel (located across the street from the Cox Convention Center) to set aside 45 rooms at a special rate for Forum attendees.
